Violations: PIC/manager/owner allowing sleep to take place in establishment as evidenced by made up cot/fold up bed in the storage room with sheets, blankets, and pillow on mattress. Living quarters are not allowed in food establishment. Cot must be removed ASAP. Lettuce in WIC in poor condition and very wilted. Lettuce at CH prep station was also brown, wilted and in poor condition.
