Violations: Handwashing sink in rear of establishment did not have hot water, soap dispenser soiled, handwashing sinks soiled, all sinks need to have towels dispensed when employees finish washing thier hands. COS, hot water turned back on by manager [inspector's note: was a comment from Mike the GM about saving money with hot water turned off] and manager promised to fix, clean and provide soap and towels in clean working dispensers. Black beetles (2) observed crawling on top of hamburger buns to be used for customer service [Inspector note: suspect Tribolium species]. A banana [cut] with excess brown oxidation observed lying on window sill next to dry and encrusted pitchers used to dispense frozen beverages and novelties. DO NOT serve food that has been adulterated by insects or that has been set on visible soiled filthy surface as you would not want to eat this yourself or serve it to your family. COS, corrected after asking twice for management to discard these foods [inspector note: bananas were originally set in cooler before being asked yet again to discard and not keep them.] Food that was over 3 months old was also discarded. Food in the freezer such as hamburger patties and sausage patties were left open and uncovered after being restocked for the lunch rush. Cover after each use. Was covered by management upon request of inspector, COS. Corrected On-Site. New Violation. Improper dishwashing procedure: dishes were observed being washed then dipped in sanitizer no rinse. Old food, grease and food debris observed not being cleaned off dishes [as evidenced by observing cleanliness of dishes on drying rack]. Food contact surface cleanliness: Pink mold-like substance and white calcification observed on dispensing nozzles (soda and ice cream). Black mold-like substance observed throughout ice machine. Knife used to cut onion had greasy debris and bread crumbs on it. Can opener observed soiled. Interior of ice cream machine soiled. Knife rack soiled. Tongs and utensils [being used to dispense food] observed soiled. Hot dog roller display soiled. Food found stored past disposal date marked: cheese dated from March 22 and March 26 observed in walk-in cooler at time of inspection- sliced swiss and cheddar. Cole slaw not labeled or dated. COS, all undated and past expiry date items discarded. Cleaner, degreasers and soiled equipment stored above and with onions in storage area. Sanitizer and floor cleaner stored above and with slicing equipment under counter next to 3-compartment sink. Hot water not available to all sinks at time of inspection. Hot water was turned off. Repairs are needed. Management stated that all sinks will be repaired ASAP. Wastewater accumulation observed under the storage racks, drain to mop sink sink clogged with built-up debris. Please clean/repair. Evidence of roach infestation (light to moderate) observed. Clean up frass and dead bugs. Live mole crickets observed in establishment. Live beetles observed in food. Please keep all pests out of the food supply. Work on a pest control plan with a license pest control operator (PCO).
